• You must be at least 18 years old 

​

• 16 and 17 year old minors must have Florida Department of Health's "Minor Tattoo Consent Form" Notarized by an official Florida Notary and signed by the Parent or Legal Guardian

• Minors must have their Parent or Legal Guardian present to witness the tattoo.

​

• Everyone must bring a Federal or State issued Photo I.D. to be scanned (NO PHOTOCOPYS OR ON THE PHONE COPIES)

​

• Parents or Legal Guardians can present birth certificates or court ordered documents showing guardianship as a form of identification for the minor.

​

• You must show up on time to the scheduled appointment time in order to retain your time slot.

​

• You MAY NOT be drunk or high during the scheduled tattoo appointment and you may be asked to leave if the artist feels you are. 

​

• Failure to present I.D. , arriving 30 minutes past the scheduled appointment time, arriving drunk or high, or acting disorderly towards Studio B Patrons or Staff means you forfeit  the ability to receive your deposit back, and rescheduling would require a new deposit.
